Condition or Practice: The cables entering and exiting the 2NS stacker disconnect and starter metal
enclosures were not passing through a proper fittings. The cables were not
provided with strain relief that would prevent them from being pulled out of
the box. Should the cable become pulled loose the exposed conductors may come
in contact with a miner or the metal enclosure causing possible electric shock
and or burns. There were no tools or materials stored near the starter. The
mine operator contracted the installation of the equipment to an electrical
company approximately 3 years prior. The mine operator was not aware of the

Action to Terminate: The mine operator had the cables clamped inside of the
enclosure, providing strain relief.

Why this concerns you: Be further advised that with respect to citation 8841972 I would like the
opportunity to explain that I think this would have been caught in a pre-season
inspection. The cable was not loose there was no danger and we should have the
opportunity to inspect our plants for such potential hazards before being cited.
