Condition or Practice: An adequate pre-operational inspection of the haul-truck was not being perfomed before being placed in operation. This was discovered during a routine safety inspection of the truck. The equipment operator did not know how to check the emergency steering. The company inspection party was also unaware of the operating procedures. After review of the operating manual and troubleshooting, the mechanic discovered that the truck had an accumulator, and the unit was defective. The mine operator is responsible to maintain and check operation of important safety features such as the steering.

Action to Terminate: All safety devices are being checked.

Why this concerns you: The operator did perform a pre-operationals inspection of the truck. He did know how to check the emergency steering. The mechanic knew the truck had an accumulator and after further checking found it to be defective. The inspector never asked the driver if he performed a pre-operational inspection.
