Condition or Practice: A guard was not provided for the belt drive system of the secondary crusher. The belt drive operates at a high rate of speed with 300 hp electric motor. The drive system is located 11 feet 4 inches above the ground with a elevated travel way that could be access with ladders in the area. This exposes a miner to the hazard of accidentally contacting moving machine parts.

The termination due date is extended to allow time for the mine operator to order the required steel to guard the drive system on the basis that is are is not accessed except for when the area is lock and tag out in accordance with company policy.

Action to Terminate: The area will be guarded or taken down.

Why this concerns you: For over a decade this work platform has passed all inspections,in fact a previous field supervisor told use that is we used signage reading "NOTICE - THIS IS A WORK PLATFORM ONLY CLEAN PRIOR TO USE AUTHORIZED PERSONNEL ONLY FOLLOW ALL LOCKOUT/TAGOUT & FALL PREVENTION PROCEDURES", this we did. But,we have been told that all work platforms will be inspected as though they were at ground level regardless of accessibility in this instance over 11 feet off the ground with no immediate access.
